LIABILITY WAIVER & BACKGROUND CHECK
Liability Waiver & Health Information Form- Any student participating in any church sponsored event will need to have this form filled out and returned to the ministry leader of the student..
Background Check - This ministry works with individuals under 18, so we require that a background check be completed for anyone working in this department.
